Assists with care management and coordination activities for patients of a primary care practice. This includes assisting with developing and monitoring care management coordination processes and support of primary clinical teams with these efforts.

Job Responsibilities
Assists to fill gaps including wellness visits and preventative screenings.
Develops and implements targeted wellness programming for attributed beneficiaries of Accountable Care Organizations (ACOs) and other contracts including employee groups.
Establishes wellness goals and monitors wellness programs that meet health outcomes, cost savings and organizational objectives.
Performs administrative or clerical duties as assigned, including filing, reception, scheduling, data entry as needed(order entry for standing orders only), patient registration, and pre-certifications.
Motivates individual and groups to take steps for good health.
Partners with other organizational and community efforts that target health concerns.
Assists with group educational offerings.

Education

High school diploma or General Equivalency Diploma (GED) preferred.

Licensure/Certification/Registration

Certified in Basic Life Support Healthcare Provider (BLS) preferred.

Work Experience

Previous experience in a medical office setting preferred.

Skills and Attributes

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
